Better to say in his chest. If Sai's thousand-year-old ghost sorrow could make Hikaru throw up repeatedly, it stood to reason that it could make Hikaru's chest feel tight and painful, as if something had bunched up inside.
Loving Hikaru's voice in this + the way he acts his age. I think the strongest part of this fic is knowing that Sai will go away, so uhm yeah. It really works.
